Before you try to find a PPO provider, refer to your ID card to find your network. Click on your logo below to begin your search. Your health plan PPO network also includes access to Mayo Clinic in Rochester, Minnesota as well as the Rochester Mayo affiliated hospital.

If you have any questions regarding your network or the providers you can access, please contact GPS at 888-301-0747. Failure to use in-network providers will greatly reduce the benefits for your claims.

NOTE: ACCESS TO TRAVEL PPO NETWORKS IS ONLY AVAILABLE FOR EMERGENCY TREATMENT WHEN TRAVELING OUTSIDE OF YOUR PRIMARY NETWORK’S AREA. Use of providers in the travel PPO network will still be considered Out-of-Network and paid at a reduced benefit level unless you meet the requirements for an Emergency/Travel exception.
